I've never felt ill from mtx, but I do have some mild fatigue. The fatigue was more pronounced, but of about 12 hours less duration when I took the whole dose at once. My doctor said I could split the dose and take half in the morning and half later in the day. She also said to be sure to take the whole dose on the same day. Now, I take half the dose in the morning and the other half with the evening meal. If I'm not engaged in an activity, I start to feel a little tired in the early afternoon, and the feeling continues until the evening of the next day.

I also take 1 mg/day of folic acid to combat the potential hair loss that mtx can cause. So far, that's working ok.

MTX is chemically _very_ similar to folic acid, as a result it is a folic acid anatgonist (aka antifolate), so you *really* (really really) need to take a Folic Acid supplement when you're on MTX. (though my specialist said not on the same day)

ie enzymes in your body will "latch onto" MTX molecules rather than the Folic Acid (1000-to-1 preference), so those processes will metaphorically "spin their wheels" rather than do their job, unless you top up your folic acid levels.

I've been taking 20mg/wk for about 4 years (much higher during the first year of disease) and I don't get sick or fatigued by that low dose. I do, however, take 2mg/day of folic acid to replace what the mtx depletes.

Pierre42, Thanks for including the citation! Since few of us are professionally qualified to give out medical opinions other than our own personal experience (which may not apply to others), it's really important we cite credible studies and credible sources to backup our statements. Very good, sir! :-)

When I was reading the wiki citation, I noticed NSAIDs, in conjunction with mtx, caused problems. I have been told to not take any NSAIDs, but with 18 drugs each day, I wasn't sure which one (or more) prevented it. It looks like mtx is at least one of the culprits. That's too bad since ipubrofen used to work real well for my joint pain.

I do the opposite of Pete, take 4 of my 6 weekly pills at night before bed and the other 2 the next morning. I usually don't feel too bad, sometimes just a little bit of a weird feeling that's hard to pinpoint. I also take 1mg. of folic acid a day. Thanks for the info on that, Pierre... not that I totally understand it, but it implies, as I thought, that hair loss is not the only consideration, but there are other reasons for it as well when taking MTX.
