I'm real sorry that you're having pain. Is the pain actually in the nose, or is it more like headache? When my disease was active I would get excruciating sinus headaches that rivaled migraines. My ENT at the time gave me a bottle of nasal spray that was basically the stuff that they numb you with before they scope your nose. Other than that he gave me a Rx for Lortab. At night I would use a microwaveable heat pac on my face. I know that's not much help. If your having masses that large and pain, I would definitely see your doctor ASAP. That's usually indicative of disease activity. If your currently being treated, then I would question if your Meds dosage is adequate enough. Certainly everyone is different, but It wouldn't hurt to speak with your doctor.

My MTX is at 20mg now , the most it's been and I seem to be getting the larger masses more since dose increased
It's definitely in my nose the pain , I have a feeling the hole in my nose has increased
I don't currently have an ENT , just my Wegs specialist . I will call his office tomorrow for some advice
    Thanks so much

Jayne, it sounds as though there is disease activity. That is my primary way to tell if I am OK or not--MASSIVE SINUS PAIN. If there is pain in the nose, and you have large crusts spilling out, keep them moist by irrigating as much as you can stand. Part of the pain could be that the crusts you are getting out are ripping off the walls of your nose and sinus and leaving raw tissue. If you keep irrigating them out, it will keep the crusts softer and make the tissue less irritated.

I had good luck with one of the small hand warming electric heating pads. (like this one) I also had three different prescriptions when my headaches were bad. Lortab for the bad days, Tylenol3 when not so bad, and I wanted to do something other than sleep, and tramadol to take during the day (no bad side effects for me).

Do find a good ENT!!! Find one that will scope your nose, remove the crusts from your nose and sinuses and keep your airway open. If the ENT knows anything about this stupid disease, he'll be willing to help you in any way he/she can. If you get an ENT that will not scope your nose and remove the crusts at every visit, or tells you that you need to use a saline nasal spray or tries to just write you a prescription and get you out the door, LEAVE! Shop around until you find one that will. A good ENT will want to see the progression of your nose as the disease regresses as your treatment progresses.

Good luck, and I do hope you get some relief soon! I really, really do know what you are going through!! It is not at all nice!!!!!

I found frequent nasal rinses helped remove a lot of the gunk that builds up and causes the pressure and pain. It doesn't do much to stop the bleeding in sinuses but removing the clots frequently does seem to help avoid the build up of bigger ones that are harder to dislodge. I add some baby shampoo and hydrogen peroxide and Alkolol to my rinses to aid in cleaning the passages. It seems to make it easier to breathe too.

I'm not currently rinsing my nose or being scoped ( if that's the word ) but just steaming every now & then .
The stuff I do manage to dislodge is so big I can understand maybe raw tissue comes away too
    Thanks guys x

I used to rinse before and after the steam treatments and it seemed to make both more effective.

I don't need to steam now, but found it very helpful before sinus rinses, back when I was getting huge monsters, too. And with some mint or other herbs in the steam water, it is very relaxing and pleasant. I can't really imagine doing that and not rinsing afterwards, since it does such a good job of loosening stuff up.

I second Mike's comments, i.e. that the symptoms you are describing may indicate that there is active disease. I was going through hell with crusting and pain a year ago now, before I was diagnosed. Once I started treatment with prednisone, the crusting and pain and infestation with "monsters" subsided pretty quickly. I also agree that you might want to consider seeing an ENT to address how you should care for your nose. I've mentioned this in a few other threads, but I was lucky enough to find an ENT with a subspecialty in rhinology. She had the tools and expertise to properly (and mostly painlessly) examine me and prescribe effective treatment. In my case, she prescribed rinses with Bactroban, which is an antibiotic, and a steroid, which acts as an anti-inflammatory. She was also able to decongest me and provide a lot of relief when I was still acutely ill. She warned me to not be very aggressive in irrigating the nose; as Mike said, you could risk further damaging the delicate tissues there.
