    Hi! I'll enter the new member forum eventually - I have some privacy concerns and don't want to say a lot about myself on the internet. However, I have a question that maybe someone here can answer: What does it mean to have a positive PR3 antibody test (2.7 on scale where less than 1 is normal) and yet the anca screen evaluation for p, c and atypical is negative. Thanks so much for any help.

    I always wondered if someone can have PR3 or MPO levels high, but no P or C ANCA positivity.
    Having antibodies means the disease is in you and your body is trying to fight it.
    PR3 typically means Granulomatosis with Polyangiitis.
    MPO typically means Microscopic Polyangiitis.
    They are both diseases called vasculitis and need to be treated quickly.

    Sometimes folks can be both P and C ANCA positive.

    I forgot to add that I am seeing a specialist soon and that local drs. think I might have WG. I just don't understand the negative part of that lab result and forgot to ask the dr. what it meant.

    Thanks for your reply. In other words, this is an unusual result? Normally the C anca would also be positive? I guess I will have to wait and see what the "expert" says

    Hi Lisa,
    Yes, normally, but not always, pr3 goes with wegener's.
    You may want to have your dr do an immunoglobulins test to see if your immunoglobulins are low.
    Make sure they check your kidney function urinalysis for protein in the urine.
    Kidneys can fail fast.

    thanks yes I know about the kidneys etc. my blood work/kidney function, sed rate and c reactive are being monitored frequently and is stable for the moment. I will ask about the immunogloblins test but looking at my paperwork I don't think I had that test. The rheumatologist didn't run that test for some reason? Odd.

    I had to ask my mom's nephrologist to run the test.
    Sometimes we have beg to get things done.

    that's why this forum is so good. People on here help you know what to "beg" for! thanks again. I did notice I had a protein electrophoresis (sp?) test which is normal thank God.

    I have never heard of a protein electrophoresis test and don't know what it is for.
    I have read of a couple cases where they were anca neg, but became positive after treatment.
    However, they never mentioned if they had a high mpo or pr3 when negative.

    that protein electroph was to rule out multiple myloma I think
